6. Family Card / Kartu Keluarga
7. Latest Diploma / Ijazah Terakhir
8. Latest Credentials / SK Terakhir
9. Old Passport / Paspor lama
10. Latest Photo 4×6 white background / Pas foto terakhir ukuran 4×6 latar belakang putih
For Male : Suite with tie, no glasses, not showing teeth and natural
For Female : Blazer, no glasses, not showing teeth and natural
